腹腔镜子宫肌瘤剔除术40例临床分析

Analysis of 40 cases of laparoscopic myomectomy

目的总结40例腹腔镜子宫肌瘤剔除术的手术经验及应用价值。方法选取因子宫肌瘤行肌瘤剔除术患者80例,随机数字表法随机分为腔镜组和开腹组,每组各40例。比较两组患者术中出血量、手术时间、排气时间、术后病率、手术并发症发生率,术后残留及术后复发情况。结果腔镜组和开腹组在术中出血量、手术并发症发生率方面比较差异无统计学意义(P>0.05);与开腹组相比,腔镜组手术时间较长,排气时间较短,术后病率较少,差异有统计学意义(P<0.05)。两组患者均无术后残留及术后复发情况。结论腹腔镜子宫肌瘤剔除术具有切口美观、创伤小、术后恢复快等优点,但尚不能完全取代开腹手术,选择恰当的病例,术者操作熟练是手术成功的关键。

Objective To analyze the experience and clinical value of laparoscopic myomectomy. Methods The clinical data of 40 LMs and 40 TAMs were analyzed.The btood loss volume, operative time,exhaust time,rate of fever post operation,postoperative complications,residue and recurrence of myoma in the two groups were compared. Results There were no statistic difference in blood loss volume and postoperative complications rate in two groups （P 〉 0.05）.The operative time in LM was lower than those in TAM,the operation time was higher in LM than those in TAM,the exhaust time in LM was shorter than those in TAM （P 〈 0.05）,and the rate of fever post operation was lower than those in TAM （P 〈 0.05）.There were no residue and recurrence of myoma in the two groups. Conclusion LM has the advantage of incision cosmetic,less lesion, and rapid postoperative recovery, but it cannot completely replace TAM. The success of LM depends on the appropriate patient and skillfull in LM.
