不同方案对治疗慢阻肺合并二型呼吸衰竭疗效对比分析研究

Contrast Analysis of the Therapeutic Effect of Different Treatment Perscriptions for Chronic Obstructive Pulmonary Diseases Complicating with Type Ⅱ Respiratory Failure

目的:探讨慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭不同治疗方案的疗效,为临床合理制定治疗方案提供参考。方法:收集我院2008年至2011年收治的120例慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭患者的住院病历,分为两组,60例对照组,给予低流量持续吸氧治疗和内科药物治疗等常规治疗,60例观察组,给予联合使用呼吸机无创正压通气与内科常规治疗。通过比较治疗后两组患者的血气、心率、呼吸频率的变化。结果:观察组的观察指标改善显著好于对照组,统计学上有显著差异(P<0.05)。结论:无创正压通气治疗慢阻肺合并Ⅱ型呼吸衰竭,能显著改善患者的临床症状和血气参数,疗效确切,值得推广。

Objective： To study the therapeutic effect of different treatment perscriptions for chronic obstructive pulmonary diseases complicating with type II respiratory failure, to provide reference for the doctors to prescribe rational treatment plan. Methods： The medical record of 120 cases of COPD complicating with type II respiratory failure were collected from 2008 to 2011. They were divided into two groups, of which 60 cases were control group and had been given conventional therapy, such as persistent low flow oxygen inspiration and drug treatment, and another 60 cases were observation group had been given both Noninvasive Positive Pressure Ventilation（NIPPV） and conventional therapyand then compared then compare the changes of the parameters of blood gas, heart rate and breathing rate after treatment. Results： The improvement of the observation group was better than that of the control group, the results had significant differences in statistics （P〈0.05）. Conclusion： Treatment with NIPPV can improve the clinical symptoms and blood gas condition, the therapeutic effect is certain and it＇s worth of spreading.
