微造瘘经皮肾钬激光碎石治疗复杂性肾结石70例报道

摘要 目的探讨微造瘘经皮肾钬激光碎石治疗复杂性结石的治疗效果。方法回顾性分析自2010年1月~2011年6月共收治136例泌尿系复杂性结石患者,136例患者随机分为治疗组与对照组,分别采用钬激光碎石术与气压弹道两种术式治疗,应用统计学方法分析两种术式在治疗效果、手术时间、术中出血量以及并发症发生率等方面进行比较。结果治疗组的手术时间为(51.6±2.9)min低于对照组的手术时间为(65.4±5.1)min;治疗组术中出血量(60.1±3.5)mL低于对照组术中出血量(93.4±2.5)mL;治疗组70例患者中经钬激光碎石技术结石总清除率88.57%;对照组66例患者中经气压弹道碎石技术结石总清除率为75.76%。治疗组并发症发生率为7.14%,对照组并发症的发生率为19.70%。结论微造瘘经皮肾钬激光碎石治疗复杂性结石,手术安全性高、创伤小、恢复快、疗效好,值得临床推广应用。 Objective To explore the micro fistula percutaneous nephrolithotomy holmium laser lithotripsy for the treatment of complicated calculi. Methods A retrospective analysis from January to 2011 since 2010 June in our department, we have treated 136 cases of urological complicated gallstone patients, 136 patients were randomLy divided into treatment group and control group, respectively by holmium laser lithotripsy and ureteroscopy two surgical treatment, application of statistical methods analysis of two kinds of operation in treating effect, operation time, operation bleeding and complications were compared with respect to. Results The treatment group in the operation time was (51.6±2.9) min lower than the control group in the operation time was (65.4±5.1) min; treatment of bleeding volume in operation (60.1±3.5) mL lower than the control group, the amount of bleeding in operation (93.4 ±2.5) mL; in the treatment group of 70 cases by holmium laser lithotripsy stone aggregate clearance rate was 88.57%; the control group of 66 patients with pneumatic ballistic lithotripsy Technology stone aggregate clearance rate of 75.76%. The complication rate was 7.14% in treatment group, the control group the incidence of complications was 19.70%. Conclusion Micro fistula percutaneous nephrolithotomy holmium laser lithotripsy for the treatment of complicated calculi operation, high safety, less trauma, quick recovery, good curative effect, it is worthy of clinical application.
