胸腰椎爆裂性骨折合并脊髓损伤的治疗体会

目的探讨胸腰椎爆裂性骨折合并脊髓损伤的治疗方法与临床效果。方法选取2010年1月～2011年12月收治的胸腰椎爆裂性骨折合并脊髓损伤患者13例,根据患者临床症状与体征采取相应的手术方法与入路,对于患者的临床疗效进行总结与分析。结果本组病例手术治疗后的Frankel分级结果为:A级1例、B级2例、C级4例、D级4例、E级2例,与术前相比差异明显(P<0.05),具有统计学意义。完全瘫痪2例患者术后均有不同程度的感觉功能恢复,以感觉恢复为主;不完全瘫的11例患者,感觉运动功能不同程度恢复9例,肌力部分增加6例。结论在胸腰椎爆裂性骨折合并脊髓损伤患者的临床治疗中,应根据患者的实际病情采取相应的手术治疗方式,以最大限度解决患者的病痛。

Objective Treatment of thoracolumbar burst fractures with spinal cord injury treatment methods and clinical effect of.Methods The hospital from January 2010 to December 2011 were treated thoraco]umbar burst fracture complicated with spinal cord injury in 13 cases, on the basis of clinical symptoms and signs to take corresponding operation method and approach for patients with clinical efficacy, summarize and analysis. Results In the group of patients after operation in treatment of Frankel classification results for： al cases, B2 cases, C4 cases, D4 cases, E2 cases, compared with preoperative differences are significant （P〈0.05）, with statistical significance. Complete paralysis of 2 patients had different degree of sensory function recovery, with sensory recovery; incomplete paralysis of 11 patients, the sensory and motor function recovery in 9 cases, muscle strength increased in 6 cases,Conclusion In the thoracic and lumbar burst fracture complicated with spinal cord injury in patients with clinicalty, patients should be based on the actual condition take corresponding operation treatment, in order to maximize the resolution of the patient＇s pain.
