舒芬太尼对高龄髋关节术后应激反应的影响

The effect of sufentanil on elderly patients with hip postoperative stress reaction

目的观察舒芬太尼对高龄患者髋关节术后应激反应的影响。方法高龄患者60例，随机分为两组，舒芬太尼组（S组）和芬太尼组（F组），术后VAS评分5分时S组以舒芬太尼（0．04～O．06）μg／kg·h微量泵匀速注射，F组以芬太尼（0．3～0．45）μg／kg·h微量泵匀速注射，比较镇痛前即刻（tO）及镇痛后2h（t1）、12h（t2）各时点血流动力学及血浆儿茶酚胺浓度。结果与镇痛前比较，两组SBP、DBP均明显下降（P〈0．05），F组镇痛后肾上腺素浓度下降幅度明显低于S组（P〈0．05）。结论对高龄术后患者进行有效的镇静、镇痛治疗，能够降低其过度应激状态，舒芬太尼对高龄患者的应激状态抑制作用强于芬太尼。

Objective To observe the effect of sufentanil on elderly patients with hip postoperative stress reaction. Methods All of 60 cases of elderly patients were randomly divided into two groups, with sufentanil group （group S ） and fentanyl group （ group F）, preoperative 5 points of VAS score in group S with sufentanil （0.04-0.06） μg/（kg·h） micro pump uniform injection while in group F （0.3-0.45） μg/（kg·h） fentanyl injection pump speed. To compare the analgesic immediately before （t0） and analgesia after 2 hours （t1）, 12 hours （t2） at different time points on hemodynamics and plasma catecholamine concentration. Results Compared with analgesia before, two groups of SBP, DBP decreased significantly （P 〈 0.05）, group F analgesia after epinephrine concentrations decreased significantly lower than that in S group （P 〈 0.05）. Conclusion On the aged patients with postoperative effective sedative, analgesic therapy, can reduce the excessive stress state, the stress state of sufentanil on aged patients with stronger inhibition to fentanyl.
