摘要 目的探讨使用胰岛素治疗2型糖尿病对并发结直肠癌肿瘤预后的影响。方法收集2型糖尿病并发结直肠癌病例共145例,并且行手术治疗。根据是否注射胰岛素治疗2型糖尿病,分为试验组(使用胰岛素组)和对照组(非胰岛素组),结合Ki-67肿瘤组织阳性表达情况、肿瘤病理分期、淋巴结累及情况、分化程度等临床病理因素进行统计学分析。结果使用胰岛素组的淋巴结转移率、术后病理分期Ⅲ+Ⅳ期的比例、Ki-67阳性率高于非胰岛素组,分别为53%比32%、57%比32%、76%比53%,差异有统计学意义(P<0.05)。结论使用胰岛素治疗2型糖尿病对同时并发结直肠癌的肿瘤预后产生负面影响。 Objective To explore whether insulin therapy influences the tumor prognosis of colorectal cancer patient with diabetes mellitus. Methods Clinical datas of 145 colorectal cancer patients with diabetes mellitus operated in Wenzhou NO 2 hospital from January 2009 to December 2011 were retrospectively analyzed. Ki-67 expressions in colorectal carcinoma tissue of 145 patients were examined by immunohistochemistry. The patient populations were divided into two groups that were received exogenous insulin and control group with no insulin . Gender,age, tumor infiltration degree, differentiation extent, Ki-67 expression, lymph nodes metastasis and pathological stage differences between the two groups were compared with ehi-square analysis. Results The positive lymph nodes metastasis、positive Ki-67 expression rates, Ⅲ + Ⅳ pathological stage in test group were higher than those in control group. They were 53%vs32% ,57% vs 32% ,76%vs 53%, respectively(P 〈 0.05). Conclusion Exogenous insulin utilization for diabetes mellitus patients with colorectal cancer had an adverse impact on the prognosis of the tumor.
