儿童特发性血小板减少性紫癜患者外周血T淋巴细胞亚群变化

Study on changes and clinical significance of lymphocyte subtypes in children with idiopathic thrombocytopenic purpura

摘要 目的研究特发性血小板减少性紫癜(ITP)患者T淋巴细胞亚群的变化,了解其在ITP发病机制中的作用。方法以30例ITP确诊患者为观察组,其中慢性ITP(CITP)组18例,急性ITP(AITP)组12例,以20例体检健康者为对照组,应用流式细胞术检测受试对象外周血T淋巴细胞亚群,并用统计学分析结果。结果 ITP组CD3+、CD4+细胞百分比及CD4+/CD8+比值比正常对照组明显减低(P<0.05),CD8+细胞百分比比正常对照组明显升高(P<0.05),CITP组变化幅度大于AITP组(P<0.05)。结论 ITP患儿存在细胞免疫功能紊乱,可能与ITP免疫发病机制有关。 Objective To study the changes of T lymphocyte subsets in peripheral blood of patients with idiopathic thrombocytopenic purpura(ITP) ,and to explore the possible pathogenesis of ITP. Methods All of 30 patients with ITP,including 18 cases with chronic ITP (CITP) and 12 cases with acute ITP (AITP), were enrolled as observation group, and 20 healthy controls were enrolled as control group. Flow cytometry (FCM) was used to detected the T lymphocyte subtype in 30 patients with ITP and 20 normal patients. Results The percentage of CD3+ T lymphocyte and CD4+ T lymphocyte and the ratio of CD4+/CD8+ in patients with ITP were obviously lower than normal patients (P〈0.05) ,but the percentage of CD8+ T lymphocyte was higher than normal patients (P 〈 0.05 ). Rangeability of these para meters was more obvious in CITP group than that in AITP group. Conclusion There are functional disorders in patients with ITP,it may be related to immune pathogenesis of ITP.
