腹腔镜胃十二指肠溃疡穿孔修补术32例疗效分析

The efficacy of 32 cases laparoscopic gastroduodenal ulcer perforation repair

目的探讨对急诊科收治的胃十二指肠溃疡穿孔患者实施腹腔镜手术的疗效。方法 62例胃十二指肠溃疡穿孔修补术患者根据术式不同分为A组32例(行腹腔镜手术),B组30例(行开腹手术),比较A组和B组的手术时间、术中出血量、胃肠功能恢复时间、疼痛程度评分、住院时间及并发症情况。结果与B组比较,A组手术时间短,术中出血量少,术后胃肠功能恢复快,且患者疼痛程度轻,住院时间短,差异有显著性(P<0.05)。A组术后并发症发生率达3.1%;B组术后并发症发生率达16.7%,差异有显著性(P<0.05)。结论腹腔镜胃十二指肠溃疡穿孔修补术具有创伤小、术后恢复快、住院时间短、并发症少等优点,值得广泛应用于急诊手术。

Objective To investigate the efficacy of laparoscopic surgery on pediatric patients hospitalized for gastroduode- hal ulcer perforation patients. Methods All of 62 cases of gastroduodenal ulcer perforation repair patients were divided into A group of 32 patients（laparoscopic surgery）, depending on the surgical control group of 30 patients（laparotomy）,the op- eration time and recovery time, blood loss,gastrointestinal function, pain severity score,length of hospital stay and com- plications were compared between group A and group B. Results Compared with group B,group A had shorter surgical time, less blood loss, postoperative gastrointestinal function recovery fast, light and the degree of pain, shorter hospital stay, the difference was significant（P 〈 0.05）.A postoperative complications occurrence rate of 3.1%;B postoperative com- plication rate of 16.7%, the difference was significant （P 〈 0.05）. Conclusion Laparoscopic gastroduodenal ulcer perfora- tion repair with trauma, rapid postoperative recovery, shorter hospital stay, fewer complications, it is worth widely used in emergency surgery.
