心房颤动合并三度房室传导阻滞心电图分析

An analysis of ECG with atrial fibrillation complicatedⅢ° A _ V bolck

为了解心房颤动合并三度房室传导阻滞的心电图诊断条件、线索及其表现形式 ,观察48例心房颤动合并三度房室传导阻滞的心电图。结果发现有10类表现形式 ,以房室交接区、室性逸搏心律或加速性逸搏心律为多 ,其次是房室交接区逸搏_室性期前收缩二、三联律及房室双层传导阻滞 ;逸搏频率<40次/min6例 ,41～60次/min33例 ,61～75次/min9例。提示心房颤动合并三度房室传导阻滞的心电图诊断条件应为完全性房室分离 ,且心室率以逸搏频率≤60次/min为准 ,而不是根据平均心室率。

To research criteria,clue and appearance of ECG diagrams in cases with atrial fibrillation complicated Ⅲ° A _ V block,analysis of ECG of 48 cases with atrial fibrillation complicated Ⅲ° A _ V block was performet.Results found:There were 10 sorts of appearance.Most seen were junctional rhythm,or ventricular escape rhythm or accelerated escape rhythm.Others were junctional escape,ventricular premature digeminy and trigeminy and A _ V double layer block.Escape rate less than 40 bpm were in 6 cases,41～60 bpm in 33 cases and 61～75 bpm in 9 cases.ECG diagnostic criteria of atrial fibrillation complicated Ⅲ° A _ V block were as follows:(1)complete atrioventricular dissociation;(2)ventricular rate should base on escape rate≤60 bpm and rather than average ventricular rate.
