日本血吸虫31/32kDa抗体的检测及其在诊断中的作用

Detection of Schistosoma Japonicum 31/32kDa Antibody and Its Value in Diagnosis for the Disease

利用纯化日本血吸虫３１／３２ｋＤａ 抗原（Ｓｊ３１／３２） 与可溶性虫卵抗原（ＳＥＡ）ＥＬＩＳＡ方法检测急、慢性日本血吸虫病血清，以探讨其作为抗原诊断血吸虫病的特异性、敏感性及疗效考核价值。结果急、慢性血吸虫病人血清抗Ｓｊ３１／３２ 抗体检出率为１００％ 和９５％ ，与正常人、肝吸虫和肺吸虫病人血清未出现交叉反应；抗ＳＥＡ 抗体检出率为９７－４ ％ 和９０％ ，与正常人和肝、肺吸虫有不同程度的交叉反应。血吸虫病人治疗后３ 个月、半年和１ 年抗Ｓｊ３１／３２ 抗体的阴转率分别为５２－５％ 、７２－５％ 和８２－５ ％ ， 而抗ＳＥＡ 抗体的阴转率则分明别为１５－０％ 、３７－５ ％ 和５０－０ ％ 。提示Ｓｊ３１／３２ 抗原诊断血吸虫病敏感性高、特异性强，并且具有较好的疗效考核价值，可用于血吸虫病现场和临床应用。

To study the specificity and sensitivity and evaluation of therapy efficacy of the purified Schistosoma japonicum 31/32 kDa(Sj31/32)antigen in the diagnosis for the schistosomiasis japonicum,schistosomiasis sera were detected with purified Sj31/32 antigen and SEA by ELISA.The positive rate of anti Sj31/32 antibody in the acute and chronic schistosomiasis japonicum were 100% and 95%,while the positive rate of anti SEA antibody were 97 4% and 90%.No cross reaction was produced by Sj31/32 antigen,while some nonspecific reaction were emerged by SEA with normal human,patients with Paragonimiasis and Clonorchiasis.The negative conversion rate of anti Sj 31/32 antibody were 52 5%、72 5% and 82 5% respectivty at 3,6 and 12 months after chemotherapy while the negative conversion rate of anti SEA antibody were 15%、37 5% and 50%.The results indicated that Sj31/32 antigen might have higher sensitivity and specificity than SEA in the diagnosis of schistosomiasis japonicum and have the value of evaluation before and after chemotherapy.It might be used in the clinical diagnosis of schistosomiasis japonicum.
