米力农治疗慢性肺心病合并心力衰竭疗效观察

Clinical Observation of Milrinone in the Treatment of Chronic Pulmonary Heart Disease with Heart Failure

目的探讨米力农治疗慢性肺心病合并心力衰竭的临床疗效。方法 56例患者入院后给予抗感染、利尿、改善呼吸功能等综合治疗,在综合治疗方案基础上,给予米力农注射液20mg,加入生理盐水30mL,微量泵泵入,观察治疗前后患者临床情况、心功能改善情况,应用多功能监护仪监测血压及心率、心律,检测血、尿常规、肝肾功能及电解质情况。结果 56例经米力农治疗后,显效14例,有效38例;治疗前后患者的血、尿常规、肝肾功能及电解质无明显变化;另3例用药过程出现心律失常,未给予特殊处理逐渐好转,1例死于严重感染合并Ⅱ型呼吸衰竭。结论慢性肺心病合并心力衰竭患者应用米力农治疗疗效满意,未观察到严重不良反应,值得临床应用。

Objective To observe the clinical curative effect of milrinone the in treatment of chronic pulmonary heart disease with heart failure. Methods After admission, all 56 patients were taken comprehensive measures of anti-infection, diuresis, improving ventilation. In addition to the comprehensive treatment scheme, all patients were given milrinone injection 20 mg, and 30 mL normal saline with a micro-infusion pump, to observe the clinical situation, improved cardiac function before and after treatment, to monitor blood pressure and heart rate with multi parameter monitor, and to monitor the blood and urine routine, liver and kidney fimction and electrolyte status. Results After the treatment with milrinone in 56 patients, 14 cases were effective obviously, 38 cases were effective. Before and after treatment, blood and urine routine, liver and kidney function and electrolyte status were not changed obviously. 3 cases appeared with cardiac arrhythmia during treatment and got better without special treatment. One case died of serious infection merged II type of respiratory failure. Conclusions Milrinone is effective in the treatment of chronic pulmonary heart disease with heart failure, which has no serious adverse reactions and deserves clinical application.
