CD44v6和MMP-9在鼻内翻性乳头状瘤中的表达及临床意义 被引量:1

摘要 目的探讨鼻内翻性乳头状瘤(NIP)中CD44v6与MMP-9的表达及临床意义。方法采用免疫组织化学SP法检测60例鼻内翻性乳头状瘤、40例下鼻甲组织、40例鼻腔鳞状细胞癌组织中CD44v6及MMP-9的表达。结果 CD44v6蛋白在鼻内翻性乳头瘤组织中呈阳性表达,阳性表达率为66.67%(20/30),定位于内翻性乳头瘤组织上皮细胞的胞浆和胞膜中;在鼻腔鳞状细胞癌组织中亦呈阳性表达,主要定位于癌细胞胞浆和胞核中,阳性表达率为30.00%(6/20);而在正常下鼻甲组织中不表达(0/20),表达率差异有统计学意义(P<0.05)。MMP-9蛋白在鼻内翻性乳头瘤组织中呈阳性表达,其阳性表达率为40%(12/30),定位于鼻内翻性乳头瘤组织上皮层上2/3的细胞胞浆及胞核中,呈棕褐色颗粒;在鼻腔鳞状细胞癌组织中亦呈阳性表达,其阳性表达率为70.00%(14/20),主要定位于癌细胞胞浆中;而在正常下鼻甲组织中呈低表达10.00%(2/20);表达率差异有统计学意义(P<0.05)。鼻内翻性乳头瘤组织中CD44v6与MMP-9表达呈正相关(r=0.494,P<0.05)。鼻腔鳞状细胞癌中CD44v6与MMP-9的表达呈负相关(r=-0.454,P<0.05)。结论在鼻内翻性乳头瘤发展、恶变为鳞状细胞癌过程中,CD44v6可能起负向调控作用,MMP-9起到重要的促进作用,CD44v6与MMP-9关系密切。同时检测CD44v6与MMP-9可提高判断鼻内翻性乳头瘤性质准确性,为临床手术方式选提供依据。 Objective To investigate expression and clinical significance of CD44v6 and matrix metaUoproteinases (MMP-9) in nasal inverted papilloma (NIP). Methods The expressions of CD44v6 and MMP-9 were detected by immunohistoehemistry (SP) in 60cases of NIP,40 cases of inferior nasal concha tissues and 40 cases of nasal cavity squamous cell carcinoma. Results The expression of CD44v6 in NIP was positive, with the positive expression rate being 66.67% (20/30) ,with the positive expression rate being 30.00% (6/20). CD44v6 was not expressed in normal inferior nasal eoncha tissues(0/20). There were significant differences among them ( P 〈0.05). The MMP-9 in NIP tissue was positive,with the positive expression rate being 40% (12/30). The expression of MMP-9 in nasal cavity squamous cell carcinoma tissue was also positive, with the positive expression rate being 70.00% (14/20). The expression of MMP-9 in normal inferior nasal eoncha tissues was lower, with the expression rate being 10.00% ( 2/20 ), there were significant differences among them ( P 〈 0. 05 ). The expression of CD44v6 in NIP tissue was positively correlated with that of MMP-9 ( P 〈 0.05 ) , however,the expression of CD44v6 in NIP tissue was negatively correlated with that of MMP-9 ( P 〈 0.05 ). Conduslon When NIP develops and caneerates into squamous cell carcinoma, CD44v6 may play an negative regulation role, however, MMP-9 may play an important promotive effect, and the expression of CD44v6 is closely related with that of MMP-9. The combined detection of CD44v6 and MMP-9 can enhance the accuracy for evaluating the nature of NIP, and can provide basis for choosing the way of clinical surgery,
