摘要
目的 通过检测血浆 ET、CGRP,观察不同全麻用药对机体内环境的影响。方法 2 4例上腹部手术病人 ,ASA ～ ,随机分为两组 ,异丙酚组 (P组 ) 12例和硫贲妥钠组 (S组 ) 12例 ,采用放免法测定诱导前、插管后、术中、术毕时的血浆 ET、CGRP、ACTH值。结果 S组 ET在插管后和术毕均较诱导前显著下降 (P <0 .0 1) ,CGRP在插管后、术中、术毕三个点都较诱导前显著性升高 (P <0 .0 5 ) ,S组和 P组 ACTH变化相似 ,插管后较诱导前显著降低 (P <0 .0 1) ,术中、术毕均较诱导前显著升高 (P <0 .0 1)。结论 异丙酚组对维持 CGRP、ET稳定相对优于硫贲妥钠组 ,两组用药均不能较满意抑制手术麻醉引起 ACTH的显著升高。
Objective To observe the influence of different general anesthetics on plasma calcitonin gene related peptide and endothelin in surgical patients.Methods 24 patients with ASA in grade Ⅰ Ⅱ were divided randomily into propfol group (P group,n=12) and sodium thiopental group (S group,n=12).Plasma ET,CGRP,ACTH in two groups in pre including and post intubating,intro and post operation periods were determind by radio immunological method,respectively.Results In S group,plasma ET in post intuvating and post operation periods decreased remarkably than that in pre inducing period (P<0,01),but plasma CGRP in post intubating,intro and post operation periods increased remarkably than that in pre inducing period (P<0.05).Plasma ACTH was similar between S group and P group.Plasma ACTH in post intubating period increased remarkably than those in pre inducing period (P<0.01),and in intro and post operation periods increased remarkably than those in pre inducing period (P<0.01).Conclusion Propfol group is comparatively superior to sodium thiopental group in maintaining the stability of CGRP and ET,but general anesthetics in two groups can’t repress effectively the marked increasement of ACTH due to operation and anesthesia.
