经伤椎椎弓根撬拨复位椎体植骨成形并内固定治疗胸腰椎爆裂性骨折156例效果分析 被引量:2

摘要 目的探讨经伤椎椎弓根撬拨复位椎体成形结合椎弓根钉棒系统内固定治疗胸腰椎爆裂性骨折的临床疗效。方法对156例胸腰椎爆裂性骨折患者采用椎弓根钉棒系统内固定治疗,同时经伤椎椎弓根撬拨复位椎体中填塞人工骨椎体成形、椎板开窗减压治疗。术后第1个月及第24~36个月追踪应用疼痛视觉评估表(VAS)评价患者胸腰部临床症状改善情况,测量伤椎体成形后椎体高度变化改善情况。结果失访4例(2.6%),152例(97.4%)获得随访24~36个月,平均32个月。患者临床症状明显好转,骨折椎体愈合良好,椎体高度恢复良好,后凸畸形无加重,无内固定物松动、断裂,无脊髓神经损伤加重情况。结论采用经伤椎椎弓根撬拨复位椎体成形并椎体内植骨内固定治疗胸腰椎爆裂性骨折能有效维持椎体的强度、高度,更好地防止后期脊柱后凸畸形的复发,减少腰背痛及其他并发症发生。
