2型糖尿病肾病患者血小板及血脂水平变化的临床研究

Clinical research on platelet and blood lipid changes of patients with type 2 diabetes and nephropathy

目的探讨2型糖尿病肾病患者血小板及血脂检测的临床价值。方法选取苏州市吴中区东山地区人民医院2010年3月至2012年6月收治的2型糖尿病肾病患者71例为观察Ⅱ组,选取该院同期无并发症的2型糖尿病患者70例为观察Ⅰ组,同期健康体检人员70例为健康对照组,分别检测各组人员的血小板及血脂水平。结果观察Ⅰ组血小板平均体积、血小板分布宽度、总胆固醇、三酰甘油、低密度脂蛋白均明显高于健康对照组,观察Ⅰ组血小板计数、高密度脂蛋白均明显低于健康对照组,差异均有统计学意义(P<0.05)。观察Ⅱ组血小板平均体积、血小板分布宽度、总胆固醇、三酰甘油、低密度脂蛋白均明显高于健康对照组和观察Ⅰ组,观察Ⅱ组血小板计数、高密度脂蛋白均明显低于健康对照组和观察Ⅰ组,差异均有统计学意义(P<0.05)。结论 2型糖尿病肾病患者伴有血小板参数变化与血脂代谢异常。及早、及时进行血小板及血脂水平检测可以为2型糖尿病肾病的临床诊断提供有效的科学依据,对疾病的预防具有重要的临床价值。

Objective To investigate the clinical value on platelet and blood lipid changes of patients with type 2 diabetes and nephropathy. Methods 71 patients with type 2 diabetes and nephropathy were selected in the hospital from February 2011 to June 2012 as the observation Ⅱ group. 70 type 2 diabetic patients without complications were selected at the same period as the observation Ⅰ group. 70 health physical examination personnel were selected at the same period as the control group. The platelet and blood lipid level were detected. Results Platelet mean volume, platelet distribution width, total cholesterol, triglyceride,low density lipoprotein in the observation I group were significantly higher than those in the control group. Platelet count, high density lipoprotein in the observation I group were significantly lower than those in the control group. Difference was statistically significant（P〈0.05）. Platelet mean volume,platelet distribution width, total cholesterol, triglyceride, low density lipoprotein in the observation Ⅱ group were significantly higher than those in the control group and the observation Ⅰ group. Platelet count,high density lipoprotein in the observation Ⅱ group were significantly lower than those in the control group and the observation group. Difference was statistically significant（P〈0.05）. Conclusion Patients with type 2 diabetes and nephropathy has the changes of platelet parameters and abnormal metabolism of blood lipid. Early and timely detection of platelet and blood lipid can provide effective and scientific basis for clinical diagnosis of type 2 diabetes and nephropathy,which has important clinical value for the prevention of the disease.
