关节镜术中止血方法探讨

Research into Hemostasis in the Course of Arthroscopy

目的 :探讨关节冲洗液加副肾素止血法在关节镜术中的止血效果及副作用。方法 :用 1mg/ml的副肾素 1支加入装有 10 0 0ml生理盐水的冲洗吊瓶 ,配成 1mg/ 10 0 0ml浓度 ,在镜下手术开始时使用 ,观察术中止血效果、血压、心率变化及术后并发症。结果 :144例 15 9膝术中止血有效率 97.49% ,优良率 81.77% ,较差 2 .5 1%。用药后血压超出正常 1例 ,心率均在正常范围 ,术后血关节发生率 9.0 3% ,无其他并发症。结论 :关节冲洗液加副肾素止血法可用于绝大多数膝关节镜下手术 ,操作简便 ,不受时限 ,使用安全 ,止血效果好 ,能有效避免止血带法所见的各类并发症。

Objective:To research into the hemostatic effect and side effect of arthro-douching liquor plus accessory suprarenin in the course of arthroscopy.Method:1mg of accessory suprarenin was added into the couching bottle with 1 000 ml of normal saline.The 1mg/1 000ml douching liquor was used at the behinning of the operation.Then,observed the hemostatic effect,the changes of BP and heart rate and post-operation complications.Result:In the 159 Knee joint operations(144 cases) the effective rate of hemostasis was 97.4%,while the rate of good result was 81.77% and poor result,2.51%.In one case,the BP exceeded normal scope after medication.All heart rates were within normal scope.The incidence rate of hemarthrosis was 9.03%.No other complications occurred. Conclusion:Hemostasis of douching liquor plus accessory suprarenin could be applied to most arthroscope operations.It is easy to perform,no time limit,safe,good hemostatic effect and could effectively avoid various complications arising from hemostasis with tourniquet.\;
