微创切口在胸部外科的临床应用

The clnical use of microtraumatic incision in thoracic surgery

妥善地选择剖胸切口 ,是减少胸部手术损伤和术后并发症的重要途径之一 ,而充分良好的术野暴露则是胸科手术成功的关键 ,我们在 1997年 8月至 1998年 12月采用了腋下微创切口成功地施行了胸腔手术 18例。其中先天性动脉导管结扎 8例 ;纵隔肿瘤切除 2例 ;肺叶切除 6例 ,贲门痉挛 2例。全部病人手术过程顺利 ,术后康复快 ,无并发症 ,痊愈出院。我们认为 ,这种不损伤胸壁大片肌肉的腋下小切口 ,术野暴露良好 ,便于操作 ,适用于多种胸科手术 ;对神经、血管、肌肉损伤小 ,有利于病人术后迅速恢复 ;切口愈合快、隐蔽 ,符合美容观念。因此 ,有较广阔的前景和较实用的价值。

Objective:Thc choice of an appropriate thoracic incision is one of the important ways to reduce the operation injury and post operative complications But sufficient exposition is the key to finish thoracic surgery successfully We had performded 18 cases of thoracic operations through microtraumatic incision successfully from August,1997 to December,1998,including 8 cases of ligation of congential PDA,2 cases of excision of mediastinum tumor,6 cases of pneumonectomy and 2 cases of cardiospasm All of the operation were succssful and without complicaions The patients recovered quickly We consider that the subaxillary microtraumatic incision,which do not injure large numbers of muscles,have many anvantages:sufficient exposition,easy operation,suitable for many thoracic operations:lesser injury to the nerves、blood vessles and muscles,which lead to the rapid recovery;Rapid healing and well concealing,which conformed to the cosmetic view So,It has wide prospects and fairly practical values
