椎板回植ARCH钢板固定治疗胸腰椎椎管内肿瘤的手术配合

Experience of surgery cooperation in thoracolumbar intraspinal tumor with replacement of posterior spinal arch and laminoplasty

目的研究胸椎腰椎椎管内肿瘤患者经过全椎板切除后回植ARCH钢板内固定的临床效果。方法对5例胸腰椎椎管内肿瘤患者进行全椎板切除,在肿瘤摘除后,再利用ARCH钢板进行全椎板回植固定并行棘上韧带修复。结果所有患者手术均按计划顺利完成,术后病痛明显减轻,甚至消失,且下肢麻木症状减轻。结论对于胸腰椎椎管内肿瘤而言,全椎板切除后回植内固定是一种安全且有助于胸腰椎稳定、防止椎管内疤痕增生的有效手术。

Objective A prospective clinical follow up study of patients who underwent thoracolumbar intraspinal surgery with replacement of the posterior spinal arch and supporting ele- ments is reported. Methods From November 2009 to Mary 2011. 5 patients underwent intraspinal surgery with osteotomy and replacement of spinal laminae using an air drill and mini- plates with repair of the supraspinous ligaments were analyzed. Results All operations were performed by the plan. Postoperation pain was decreased apparently, even disappeared, the lower limb numb symp- tom was relieved. Conclusion This technique is safe and well - suited to serve as a standard poste- rior approach to intraspinal pathologies and offers distinct advantages over laminectomy and repeat surgery.
