摘要
目的 报告采用自锁式梭形植骨块在颈椎前路减压术中的效果固定。方法 对 36例脊髓型颈椎病及颈间盘突出症实施彻底前路减压后 ,将减压骨窗两端椎体凿出圆台形小槽 ,然后将梭形自体髂骨骨块上端先放入上端小糟内 ,牵引头部。下端加力夯入下端小凹窝内。术后用颈椎围领固定。结果 随访 0 8~ 10年 ,平均 5 3年 ,均无移植骨块移位、脱出。术后 3个月复查X线片 ,见植骨块无移位、脱出。术后 3个月复查植骨块完全融合。结论 梭形植骨块植入后 ,具有稳定性强、抗屈伸、抗旋转和剪力应力。由于牢固稳定 ,使植骨块愈合快 。
Objective This article reported the effect of auto-locking fusiform bone graft used in anterior cervical decompression.Methods Chiseled conical grooves on the vertebrae upper and lower ends of the rectangular slot after completely decompression by means of excising cervical disc and removing the hyperplastic bone which had pressed spinal cord using rose drill and trepan in 36 patients.Placed upper end of self-fusiform-coxae into upper conical groove,drawed the head and pressed lower end of bone graft into conical groove.Fixed neck with cervical vertebral loop after operation.Results followed from 0 8 to 10 years,average 5 3 years,there were not displacement and herniation of bone graft in all 36 cases.Bone graft located proper position on X-ray after operation 3 weeks and fused completely after operation 3 months.Conclusions The fusiform bone graft embedded had strong stability,could resist flexion-extension and reduce rotation-shearing strss.Bone graft fused quickly due to strong stability and avoided complication of dislocated bone graft to press spinal cord.
