摘要
目的通过研究血浆8-异前列腺素(8-iso-PGF2α)在阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患儿中的变化探讨OSAHS的病理生理机制。方法儿童OSAHS患者51例,健康对照组24例,采用酶联免疫吸附法(ELISA法)检测两组研究对象血浆8-iso-PGF2α水平并进行比较分析。结果儿童OSAHS组血浆8-iso-PGF2α水平升高(2.5±0.9μg/L),与健康对照组比较有统计学意义(t=-4.639,P<0.01);OSAHS儿童组的血浆8-iso-PGF2α水平与其呼吸暂停低通气指数(AHI)、最低血氧饱和度SaO2(%)、睡眠呼吸障碍事件最长时间(s)、睡眠呼吸障碍事件总时间占睡眠时间百分比(%)在数量上存在依存关系,具有统计学意义(r2=0.150,P<0.01;r2=0.198,P<0.01;r2=0.217,P<0.01;r2=0.272,P<0.01);且儿童OSAHS组,其血浆8-iso-PGF2α及AHI与其伴随的变应性鼻炎呈相关性(χ2=4.173,P<0.05;χ2=10.311,P<0.05),与夜间憋醒、咽痛、扁桃体肥大、腺样体肥大无明显相关性(P均>0.05)。结论氧化应激水平的升高在OSAHS患儿发病中起重要作用,而变应性鼻炎更加重OSAHS的发生及体内氧化应激水平。
Objective To explore the pathophysiology mechanism of obstructive sleep apnea - hypopnea syndrome (OSAHS) by researchting the changes of plasma 8 - iso - proataglandin F2α (8 - iso - PGF2α ) level in children with OSAHS. Methods All observed subjects were divided into 2 groups : control group ( n = 24 ) , OSAHS group ( n = 51 ). Plasma 8 - iso - PGF2α levels were measured by ELISA and were compared. Results The plasma 8 - iso - PGF2α level were higher in OSHAS children than those in control subjects(2.5 ± 0. 9μg/L vs 1.6 ± 0.6μg/L,t = - 4. 639, P 〈 0.01 ). Also, there was a dependent relationship between plasma 8 - iso - PGF2α level and AHI ,SpOEnadir( % ) ,the longest time of sleep disordered breathing(s) ,the percentage of time take place sleep disordered breathing event(% ) in OSAHS group(r2 =0. 150,P〈0.01;r2 =0.198,P〈0.01;r2 =0.217,P 〈0.01;r2 =0.272,P 〈0.01). In children with OSAHS, there was a correlation between plasma 8 - iso - PGF2α level and AHI in the patients with allergic rhinitis (χ2 = 4. 173, P 〈 0.05 ; χ2 = 10.311 ,P 〈 0.05 ) , but no statistically significant between plasma 8 - iso - PGF2α level and awake at night, pharyngalgia, hypertrophy of tonsils,adenoid hypertrophy. Conclusion The increased oxidative stress level has a significant role in children with OSAHS, and aller- gic rhinitis further aggravated the happen of OSAHS and oxidative stress level in vivo.
