早期乳腺癌前哨淋巴结活检对预测腋窝淋巴结状态的临床探讨

摘要 目的探讨前哨淋巴结活检(sentinel lymph node biopsy,SLNB)对早期乳腺癌患者腋窝淋巴结有否转移状况预测的临床应用价值。方法对30例乳腺癌患者应用美蓝(用2mL注射器,于乳晕表面注入美蓝约2mL,如已行活检术,则在肿瘤残腔周围注入)染色寻找SLNB,并接着行包括腋窝淋巴结清扫(anxillary lymph node dissection,ALND)在内的根治性保乳手术,并分析SLNB与ALND术后的病理切片结果。结果 30例中,发现SLN29例,平均花时6min,检出率96.7%(29/30),共检出前哨淋巴结68枚,平均2.26枚,本组前哨淋巴结检出灵敏度97%(28/29),准确性97%(28/29),假阴性率3.3%(1/30),假阳性率0。结论早期乳腺癌前哨淋巴活检是一种简便、安全的检测技术,可以预测早期乳腺癌患者腋窝淋巴结的状态。 Objective The study is designed to investigate whether the sentinel lymph node biopsy (SLNB) has clinical value of predicting whether axillary lymph node transfer or not of early breast cancer patients. Methods Using methylene blue to stain and then find SLNB on 30 cases of breast cancer patients (Using 2 mL syringe,injecting about 2ml methylene blue on the surface of the areola, if used biopsy technique, then inject around the Tumor remnants cavity). Then go on with the radical breast surgery which is including anxillary lymph node dissection (ALND) and then analyze the SLNB and ALND after the biopsy results. Results 29 cases of the 30 cases are fmded SLN in an average time of six minutes, the rate of detection is 96.7% (29/30), 68 sentinel lymph node with average of 2.26 are detected,this group of sentinel lymph node detection sensitivity is 97% (28/29) in an accuracy of 97% (28/29), the false negative rate of 3.3% (1/30), the false positive rate is 0. Conclusion SLNB of early breast cancer biopsy is a simple, safe detection technology that can predict the status of axillary lymph node of early-stage breast cancer patients with.
