半椎板入路显微手术在椎管内肿瘤中的临床应用

Clinical application of the unilateral hemilaminectomy approach for microsurgical treatment in the intraspinal tumors

回顾性分析经半椎板切除手术治疗椎管内肿瘤90例的临床资料,与同期经全椎板切除手术治疗椎管内肿瘤85例进行对照比较。对手术前后的医学影像学及临床资料进行分析,评价患者的脊柱稳定性及临床疗效。半椎板组脊柱稳定性良好,均未发生脊柱畸形,全椎板组11例脊柱稳定性发生变化(P<0.05)。临床近期疗效均较满意,远期临床疗效半椎板组优于全椎板组,差异有统计学意义(P<0.05)。

To analize clinical data of 90 patients with intraspinal tumors surgically treated through hemilaminectomy retrospectively;85 patients with intraspinal tumors removed through total laminectomy approach during the same period were chosen as control group.The radiographic and clinical data were reviewed.The short and long-term follow-up was performed to evaluate spinal stability and clinical efficacy.The spinal stability was excellent and there was no spinal deformity in the hemilaminectomy group.There were 11 patients with spinal stability changed in the total laminectomy group（P0.05）.The short-term clinical efficacy was satisfactory in the two groups.During the follow-up,the hemilaminectomy group was superior to the total laminectomy group in terms of long-term clinical efficacy.The difference was statistically significant between the two groups（P0.05）.
