皮肤扩张术在治疗面颈部瘢痕中的应用及随访观察

Application and follow-up of skin expansion in the treatment of faciocervical scar

目的:探讨皮肤扩张器在面颈部瘢痕治疗中的临床应用,观察远期治疗效果。方法:手术分两期:1期于瘢痕附近正常皮下置入皮肤扩张器,充分扩张皮肤,2期行扩张器取出、瘢痕切除、皮瓣转移。结果:18例患者共置入扩张器29个,扩张器并发症6处(20%),所有患者均得到有效治疗,修复效果满意,长期随访中部分患者有更精细的要求。结论:皮肤扩张器修复面颈部瘢痕效果良好,需要更精细的操作满足患者要求。

Objective To discuss the clinical application of skin expanders in the therapy of faciocervical scars, and observe the long-term effects of the treatments. Methods Surgerys were devided into two phases. During phase 1, skin expanders were placed in normal subcutaneous near scars to make the skin expanding fully. During phase 2, after skin expanders were removed and scars were resected,the expanded skin flaps were transplanted to cover the defects. Results 29 expanders were placed in 18 patients,and there were 6 （20%） complications;all patients received effective treatment and were satisfied with the results of repairments. While during the long-term follow-up, some patients were dissatisfied and had more sophisticated requirements. Conclusion The effects of skin expanders to repair the faciocervical scars are good, while more sophisticated operations are required to meet patients.
