红斑型天疱疮误诊探讨

Probe into misdiagnosis of a pemphigus erythematosus

目的:报道面部红斑型天疱疮误诊1例并复习相关文献。方法:采用皮肤组织病理检查及其免疫荧光检查。结果:皮肤组织病理检查示表皮棘细胞间水肿,表皮内裂隙及棘刺松解细胞;真皮浅层血管扩张、充血,周围见淋巴细胞及浆细胞浸润;免疫荧光显示IgG在角质形成细胞间呈网状沉积。结论:临床表现结合皮肤组织病理及其免疫病理符合红斑型天疱疮的诊断。

Objective To report a case of misdiagnosed pemphigus erythematosus on face and review the literature related to it. Methods Histopathologic examination and immunofiuorescence were adopted. Results Histopathologic examination showed edema between acanthocytes.There were fissure of intraepidermis and acantholytic cell.The superficial layer of dermis showed dilated capillaries, congestion and infiltration of lymphocyte and plasma cell. Immunofiuorescence showed that IgG was deposited between keratinocytes. Conclusion The clinical manifestation, histopathologic features and immunofluorescence were compatible with Senear-Usher syndrome.
