摘要
目的:探讨人类β-防御素2(hBD-2)与口腔黏膜疾病发病机制的关系及其在口腔黏膜中的作用。方法:采用免疫组织化学染色法,检测hBD-2在正常口腔黏膜、口腔黏膜下纤维化(早、中、晚期)、扁平苔藓和口腔鳞癌(1级)中的表达情况,计算阳性表达率并测量免疫组化染色的平均光密度值(MOD),应用SPSS17.0统计软件进行数据分析,计数资料采用χ2检验。结果:hBD-2在NOM、OSF、OLP、OSCC中均有表达,主要表达于上皮细胞、间质细胞和基底细胞胞浆膜上。hBD-2阳性率排序为OSCC>OSF(早期)>OLP>NOM。OSCC的阳性率明显高于后三者,差异具有统计学意义(P<0.05);OSF(早期)、OLP、NOM阳性率差异不具有统计学意义,但OSF中期hBD-2表达明显上调(P<0.05)。hBD-2平均光密度值由大到小排序为OSCC>OSF(中期)>OSF(晚期)>OLP>OSF(早期)>NOM。OSCC与NOM、OSF(早、中、晚期)、OLP间差异均具有统计学意义(P<0.05),NOM与OSF早期以及OLP差异不具有统计学意义。结论:hBD-2在正常口腔黏膜、口腔黏膜下纤维性变、口腔扁平苔藓、口腔鳞癌的表达及分布情况不同。hBD-2在OSF各期中表达程度不同,可能提示hBD-2与纤维化进程有关。
Objective:To explore the effects of human beta-2 defensin(hBD-2) through studying the expressions and locations of hBD-2 in oral submucous fibrosis(OSF),oral lichen planus(OLP) and oral squamous cell carcinoma(OSCC).Methods:The expression of hBD-2 protein was examined by immuno-histochemistry method in 30 cases of OSF,including 10 cases of early,middle and late stage respectively,10 normal oral mucous(NOM),15 cases of oral squamous cell carcinoma(OSCC,grade 1),15 cases of oral lichen planus(OLP).The different positive cell rates of hBD-2 in the five groups and immunohistochemical stains average gray values were compared by statistics analysis.Results:There were positive expressions of hBD-2 in NOM,OSF,OLP and OSCC.Brown staining was mainly located in epithelial cells,interstitial cells and membrane of the basal cells.The sort of positive cell rates and expression intensity of hBD-2 among OSF,OLP,OSCC and normal oral mucous were OSCCOSF(early stage)OLPNOM.The positive cell rate of OSCC was significantly higher than the others and there was statistically significant(P0.05).There was no significant difference among OSF(early stage),OLP and OSCC,but the expressions of hBD-2 in the middle stage of OSF was distinct higher.The sort of the average gray values of hBD-2 by large to small order was OSCCOSF(middle stage)OSF(late stage)OLPOSF(early stage)NOM.There was significant difference between OSCC and NOM,OSF(early,middle and late stage),OLP(P0.05).There was no significant difference between NOM and OSF(early stage),OLP.Conclusions:There were differences in the expressions and locations of hBD-2 among NOM,OSF,OLP and OSCC.The differences in the expressions of hBD-2 in three stages of OSF may imply there was relationship between hBD-2 and the course of fibrosis.
