老年急性结石性胆囊炎手术治疗临床分析

Clinical analysis of surgical treatment of acute calculous cholecystitis in elderly patients

目的分析手术治疗老年急性结石性胆囊炎的方式及时机选择。方法选取我院2010年5月～2012年5月收治的老年急性结石性胆囊炎患者60例,按随机化原则分为观察组30例和对照组30例,观察组患者确诊后立即施行胆囊切除术,对照组则是在药物治疗后行手术治疗,比较两组患者的治疗情况。结果手术治疗后,4例患者死亡,19例患者效果良好,37例患者痊愈;所有患者都无脏器或胆道损伤;观察组的术后疼痛时间、住院时间、手术时间、术后下床活动及肛门排气时间均较对照组短(P<0.05),除死亡患者4例外,其余56例患者皆治愈。结论老年急性结石性胆囊炎手术治疗安全有效,一经诊断应立即手术治疗,不仅可以降低患者死亡率,提高治疗有效率,而且可提升患者的生活品质。

Objective To analyze the way and time choosing of surgical treatment of acute calculous cholecystitis in elderly patients. Methods Sixty elderly patients with acute calculous cholecystitis who were in our hospital from May 2010 to May 2012 were divided randomly into observation group and control group. The observation group was treated with cholecystec- tomy immediately after diagnosis and the control group was treated with surgery after medication treatment. The clinical ef- fects of the two groups were compared. Results After surgical treatment, 4 patients died, 19 patients achieved satisfactory results, 37 patients were cured; All patients had no organs or bile duct injuries; The response rate of control group was 90% and that of observation group was 96.7%. Except 4 patients died, the rest 56 patients were all cured. Conclusion Sur- gical treatment of acute calculous cholecystitis in elderly patients is safe and effective and should be adopted at once after diagnosis. It can not only reduce mortality and improve effective rate but also improve the patients＇ life quality.
