38例腹型过敏性紫癜临床分析

Clinical analysis of 38 patients with abdominal type allergic purpura

目的探讨腹型过敏性紫癜的临床表现特点，以提高临床腹型过敏性紫癜的诊断水平。方法回顾性分析38例腹型过敏性紫癜患者病历资料。结果17例（44．7％）于发病前1～7d有明确诱因；7例（18．4％）以消化道症状首发，可早于皮损2～21d出现；38例均出现腹痛；14例（36．8％）腹泻，14例（36．8％）呕吐，10例（26．3％）排黑便，4例（10．5％）排血便，4例（10．5％）呕血。结论腹型过敏性紫瘢缺乏早期特异性临床表现，应提高对本病的认识，结合其临床、辅助检查及内镜特点，及时进行早期诊断和治疗。

Objective To explore the clinical characteristics of manifestations of abdominal type allergic purpura so as to improve the level of clinical diagnosis of it. Methods The data of clinical manifestations of 38 patients with abdominal type allergic purpura were collected and analyzed retrospectively. Results A total of 17 cases （44.7%） were confirmed to have dear incentives, 1-7 days before the onset of allergic purpura. In 7 cases （ 18.4% ）, gastrointestinal symptoms were the initial manifestations, 2-21 days earlier than the skin lesions. All the 38 patients had abdominal pain, 14 patients （36.8%） had diarrhea, 14 patients （36.8%） had vomiting, 10 patients （26.3%） had melena, 4 patients （10.5 % ） had bloody stool, and 4 patients （10.5 % ） had hematemesis. Conclusions Abdominal type allergic purpura has little early specific clinical manifestations, therefore we should timely diagnosed and treated it in the early stage through improving the understanding of this disease, combined with its clinical manifestations and auxiliary examinations including understanding its endoscopic characteristics.
