摘要
目的研究早期糖尿病患者血糖波动对尿微量白蛋白的影响。方法选择2010年6月—2012年5月在该科住院的患者64例,按糖化血红蛋白水平分成3组:血糖控制良好组、血糖控制一般组、血糖控制不良组,免疫比浊法测定尿微量白蛋白水平;以平均血糖标准差(SDBG)值表示血糖波动,研究糖化血红蛋白和血糖波动水平对尿微量白蛋白的影响。结果血糖控制不良组患者尿微量白蛋白高于血糖控制一般组及血糖控制良好组(P<0.01);SDBG>2 mmol·L-1患者尿微量白蛋白均高于SDBG≤2 mmol·L-1者(P<0.05)。结论早期糖尿病患者糖化血红蛋白水平越低,尿微量白蛋白越少;相同糖化血红蛋白水平下,血糖平均波动水平越小,尿微量白蛋白越少。
Objective To clarify the influence of the blood glucose fluctuations on urinary microalbumin in early diabetes patients.Methods From June 2010 to May 2012,a total of 64 subjects recruited in this study were divided into 3 groups based on the HbAlc level:satisfied,normal,and poor glycemic control.The microalbumin urine was determined on full-automatic biochemistry analyzer with immune turbidimetry,the blood glucose fluctuations were defined as standard deviation of blood glucose by continuous glucose monitoring system.Results the microalbumin urine level was higher in the poor glycemic control cases than that in the normal or satisfied glycemic control ones(P0.01).For the cases of blood glucose fluctuations above 2 mmol·L-1,the microalbumin urine was higher than those of blood glucose fulctuation below 2 mmol·L-1(P0.05).Conclusion The early diabetes patients with lower of HbA1c level and a stable blood glucose fulctuations show a decrease in microalbumin urine.
