双侧子宫动脉化疗栓塞联合宫腔镜治疗子宫瘢痕妊娠临床分析

Clinical analysis of bilateral uterine arterial chemoembolization combine to hysteroscopy in the treatment of cesarean scar pregnancy

目的探讨双侧子宫动脉化疗栓塞联合宫腔镜治疗子宫瘢痕妊娠的临床应用价值。方法对本院52例子宫瘢痕妊娠的患者进行双侧子宫动脉内灌注甲氨蝶呤(MTX)和明胶海绵屑栓塞,3～5d后在宫腔镜下行清宫术。结果 52例患者手术均一次成功。实施双侧子宫动脉化疗栓塞中未见出血,术后清宫时出血量平均约20mL,无严重并发症。随访2～6个月,月经均恢复正常。结论双侧子宫动脉化疗栓塞联合宫腔镜治疗子宫瘢痕妊娠具有安全、创伤小、疗效好等优点,术后长期随访无严重并发症发生,值得临床推广使用。

Objective To investigate the clinical application of bilateral uterine arterial chemoembolization combine to hysteroscopy in treating cesarean scar pregnancy.Methods Fifty-two cesarean scar pregnancy patients were treated with bilateral uterine arterial chemoembolization of MTX and gelatin sponge dust,3-5 days later was treated with hysteroscopy.Results The technical success was achieved in 52 cases.There was no bleeding in the prcess of uterine arterial chemoembolization,the amount of bleeding in the process of hysteroscopy was about 20 mL,there was no serious complications.The menstruation period was recovery to normal after 2-6 months.Conclusion The bilateral uterine arterial chemoembolization is a safe and effective treatment for cesarean scar pregnancy with no serious complications,therefore,this technique is worth being used in clinical practice.
