心电图诊断左室肥厚的可靠性分析

Reliability analysis of the ECG in diagnosis of left ventricular hypertrophy

目的研究并探讨心电图对于左心室肥厚诊断的可靠性。方法以本院收治的200例高血压患者为研究对象,并以患者在进行超声心动描记术的检查结果为主要依据,将患者分为有左心室肥厚组与无左心室肥厚组,进而计算心电图国内标准与Cornell电压标准在诊断左心室肥厚中的敏感性与特异性。结果心电图国内标准对于左心室肥厚患者诊断的敏感性为23%,特异性为97.5%,而Cornell电压标准对于左心室肥厚患者诊断的敏感性为33%,特异性则为94.0%,P<0.05,两组诊断方式在对于左心室肥厚患者诊断的敏感性方面差异具有统计学意义,在诊断左心室肥厚的时候,若将二者联合使用,则对于左心室肥厚患者的检出率约为38%。结论心电图国内标准在运用的时候简便易行,但敏感性不强,而Cornell电压标准在诊断的时候操作繁琐,敏感性较强,二者的联合使用对于提高左心室肥厚患者的检出率具有显著的意义。

Objective To study and explore the reliability of the ECG for the diagnosis of left ventricular hypertrophy.Methods Two hundred hypertensive patients in our hospital were selected and divided into left ventricular hypertrophy group and no left ventricular hypertrophy group according to ultrasound echocardiography findings,and then sensitivity and specificity of electrocardiogram domestic standards and Cornell voltage criteria in the diagnosis of left ventricular hypertrophy were calculated.Results The sensitivity of the ECG domestic standards for the diagnosis of left ventricular hypertrophy was 23%,the specificity was 97.5%,sensitivity of Cornell voltage criteria for the diagnosis of left ventricular hypertrophy was 33%,specificity was 94.0%,the difference was significant （P 0.05）,the sensitivity of two methods in the diagnosis of left ventricular hypertrophy was significant different,but when the two methods combined,the detection rate of left ventricular hypertrophy was about 38%.Conclusion The ECG domestic standards is simple for use,but the sensitivity is not high,and operation of Cornell voltage criteria in the diagnosis is complicated,sensitivity is high,the combination of two methods has a significant meaning in improving the detection rate of left ventricular hypertrophy.
