摘要
目的:对邓铁涛教授治疗冠心病药物进行统计分析,为临床用药提供参考。方法:通过对邓老治疗冠心病的医案进行收集,将符合纳入标准的80例医案以统计软件SPSS13.0进行聚类分析及相关统计,得出邓铁涛教授治疗冠心病的临证经验。结果 (:1)通过聚类分型,邓老治疗冠心病的核心用药为:党参、白术、茯苓、甘草、法夏、丹参、竹茹、枳壳、桔红、五爪龙、田七末;(2)邓老治疗冠心病用药以益气健脾除痰化瘀为主;(3)80例病案总共使用了药物96种,计910药次,平均每方用药数为11.4种。结论 (:1)邓铁涛教授对冠心病病因病机的特色以正虚为本,邪实为标;五脏相通,心脾相关;痰瘀相关,以痰为主。(2)在治疗上突出了扶正,即以标本兼顾,益气为主,痰瘀为次;心脾相关,调脾护心,重在健脾;痰瘀相关,痰瘀同治,重在调气;痰瘀同治,化痰为主,祛瘀为次。
Objective:To analyze the medication laws of professor DENG Tietao treating coronary heart disease,providing some reference for clinic.Method:By collecting the medical records of Professor DENG Tietao treating coronary heart disease(CHD),and cluster analysis on the medical records of 80 patients which met the inclusion criteria was made by statistical software SPSS13.0,to obtain the clinical expertise of Professor DENG treating CHD.Results:(1)By cluster analysis,it found the key medication drugs that Professor DENG treating CHD were Dangshen,Baizhu,Fuling,Gancao,Banxia,Danshen,Zhuru,Zhiqiao,Juhong,Wuzhualong,Tianqi.(2)The methods of replenishing qi,invigorating spleen,removing phlegm and blood stasis were the key processes of Professor DENG in treating CHD.(3)A total number of 96 drugs,910 medical frequencies were used in the 80 medical records,11.4 herbs were taken in each prescription in average.Conclusion:(1)As for the etiology and pathogenesis of CHD,the main process which Professor DENG Tietao focused on were deficiency of Qi and excess of evils,five organs were related,heart was directly combined with spleen.Phlegm and stasis were combined,but phlegm played a key role.(2)As for medication,he emphasized on treating Qi,namely firstly treating deficiency of Qi,then removing phlegm and blood stasis;as for the heart and spleen,both regulating the spleen and protecting the heart,focusing on strengthening the spleen;phlegm and stasis were related,and both treating phlegm and blood stasis,focusing on phlegm,and then removing blood stasis.
