比较盈源与贺斯在失血性休克容量复苏中对凝血功能影响

Contradistinction on effect of Yingyuan and HES on blood coagulation of volume resuscitation in hemorrhagic shock

摘要 目的观察国产(盈源)与进VI(贺斯)6%羟乙基淀粉200/0.5氯化钠注射液在失血性休克容量复苏中的扩容效果及对凝血功能的影响。方法选取114名失血性休克患者,患者无贫血、凝血障碍及严重心肝肾功能不全等基础疾病,随机分为对照组进口6%羟乙基淀粉200/0.5贺斯组和观察组国产盈源组。液体复苏后分时间段检测患者的红细胞变化HB、Hct以及常规凝血指标PLT、Fib、PT、APTT、TT值,Sonoclot凝血及血小板功能分析仪测初期凝血形成时间(ACTonset)、凝血速率(ClotRate)和血小板功能fPFl。结果两组患者随着扩容时间的变化HB、PLT、CR、Fib值均有下降,PT、AFIT、TT、ACT值均有明显延长,PF降低,但组内差异无统计学差异。且两组间比较差异无统计学意义。结论盈源与贺斯在容量复苏中的扩容效果以及对凝血功能的影响无明显差异。 Objective To contrast the effect of Yingyuan (hydroxyethyl starch 200/0.5 sodium chloride injection) to HES on expansion and blood coagulation in a volume resuscitation in hemorrhagic shock. Methods 114 hemorrhagic shock patients with no anemia, clotting disorders, severe heart, liver failure, nor other underlying disease were randomly divided into two groups for Yingyuan and HES. The hemodynamic data (HB) and coagulation function (PLT, Het, Fib, PT, APTT, TT) were measured in different times after fluid resuscitation was performed. The time of the initial clotting formation (ACT onset), coagulation rate (Clot Rate) and platelet function (PF) were measured for Sonoclot coagulation and platelet function analyzer. Results HB, CR, PLT, Fib values of both groups were progressively decreased. PT, APTF, TT, ACT values were longer, and PF was decrease with the change of expansion time, with no significant difference found between the two group. Conclusion There is no significant difference between the Hydroxyethyl Starch (200/0.5) and Sodium Chloride Injection (Yingyuan) found in expansion effect of volume resuscitation, nor in blood coagulation
