摘要
目的探讨小切口劈核法白内障摘除术治疗老年性硬核性白内障的临床疗效。方法将240例老年性硬核性白内障患者纳入研究,按入院的先后顺序,随机分为观察组(120例,141眼),与对照组(120例,130眼)。观察组行小切口劈核法白内障摘除术而对照组行超声乳化白内障摘除术。术后随访3月,对术后视力大于0.4、角膜水肿及术后30d角膜散光等临床指标进行比较。结果观察组和对照组术后视力大于0.4的比率分别为:术后第3天为63.16%和65.35%;第14天为72.18%和73.23%,第90天为76.70%和74.02%,组间比较差异没有统计学意义(P>0.05)。术后30d角膜散光分别为(0.89±0.72)d和(0.86±0.67)d,组间比较差异无统计学意义(P>0.05);角膜水肿分别为21.99%和56.92%,组间比较差异有统计学意义(P<0.05)。结论小切口劈核法白内障摘除术治疗老年性硬核性白内障与超声乳化白内障摘除术的疗效近似、但其手术方法更简单经济、适合在基层医院开展应用。
Objective To explore the clinical efficacy of small incision nucleus fragmentation method in the treatment of senile hard nuclear cataract. Methods According to the order of admission,240 elderly patients with hard nucleus cataract were randomly divided into observation group(n = 120,141),and the control group(n = 120,130).The observation group was given small incision nucleus fragmentation method,while the control group underwent phacoemulsification cataract surgery,postoperative follow-up in March.Visual acuity greater than 0.4,and compare clinical indicators such as corneal edema and postoperative 30d corneal astigmatism. Results The visual acuity of observation group and control group ratio of greater than 0.4 were: 63.16% and 65.35% after 3d;14 d of 72.18% and 73.23%,76.70% and 74.02% in the first 90d group difference was not statistically significant(P0.05).30 days after surgery,corneal astigmatism,respectively(0.89 ± 0.72) D(0.86 ± 0.67) D,the difference was not statistically significant between groups(P0.05);the corneal edema 21.99% and 56.92%,respectively,and the difference was not statistically significant between groups(P0.05). Conclusions The efficacy of the small incision nucleus fragmentation method in treatment of senile hard nuclear cataract with phacoemulsification cataract extraction approximate,but the surgical method is more simple and economical,suitable for application in the primary hospital.
