影响鼻窦显微手术疗效因素的分析

THE ANALYSIS OF THE FACTORS CORRELATING THE CURATIVE EFFECTS OF NASAL SINUS MICRURGY

目的 分析影响鼻窦显微手术疗效的因素。方法 鼻内筛窦手术 ;经上颌窦手术 ;上颌窦窦口扩大 ;中鼻道开窗。结果 3 5 9侧鼻窦显微手术 ,术后 1～ 4年 (平均 3 6.4个月 ) ,随访 2 88侧 ,治愈 10 8例 ( 3 7.5 % ) ,显著进步 87例 ( 3 0 .2 % ) ,好转 65侧( 2 2 .6% ) ,无效 2 8侧 ( 9.7% ) ,良好率 67.7%。鼻内筛窦手术良好率 60 .6% ,经上颌窦手术良好率 72 % ,上颌窦窦口扩大未闭良好率60 .4% ,中鼻道开窗未闭良好率 92 .2 % ,两组相差有显著差异性 ( P<0 .0 5 )。结论 作者认为应以术后 3年以上的远期疗效作为分析比较。对全副鼻窦炎、多发性如复发性息肉 。

Objective To study the factors correlating the curative effects of nasal sinus micrurgy. Methods Intranasal ethmoidectomy, transmaxillary sinus surgery, ostium enlargement of the maxillary sinus middle meatal anstrostory are used. Results 288 of 359 patients who had sinus micrurgy were followed up for 1 to 4 years, 108 of whom recovered,87 improved significantly(32.2%), 65 improved(22.6%)and 28 had no reaction.(9.7). The effective rate of this treating method is 67.7%. The effective rates of the other treating methods intranasal ethmoidectomy, transmaxillary sinus surgery, ostium enlargement of the maxillary sinus and middle meatal antrostomy are respectively 60.6%,72%,60.4% and 92.2%. Both the groups had significant difference.(P<0.05) Conclusion Through analyzing and comparing the curative effects of the above methods the patients got over three years after they had the operatiions, the author concludes that the patients suffering from recurve all sinusitis and polypus should accept transmaxillary sinus surgery and middle meatal antrostory.
