肝动脉灌注LAK/IL-2及化疗药物治疗晚期肝癌

INFUSION OF LAK/IL- 2 INTO HEPATIC ARTERY AIDED WITH CHEMOTHERAPEUTIC DRUGS FOR THE TREATMENT OF LATE STAGE LIVER CANCER

目的 观察经皮肝动脉插管化疗药物灌注、栓塞术联合 LAK/IL -2灌注治疗中晚期肝癌的疗效、T淋巴细胞改变及副反应。方法 96例经确诊的中晚期肝癌 ,随机分为动脉化疗栓塞联合 LAK/IL -2观察组及单纯介入化疗栓塞组各 48例。两组均采用经皮穿刺肝动脉插管灌注化疗药物及栓塞。化疗药物 5 -FU 10 0 0 mg+卡铂 3 0 0 mg+表阿霉素 3 0 mg或丝裂霉素 10～ 2 0 mg,栓塞剂为碘化油和明胶海绵 ,介入治疗 3～ 4周 1次 ,2次为 1疗程。观察组在栓塞后沿导管灌注同种异体 LAK细胞 1× 10 9/10 0 ml及 IL-2 40万U,2次介入间隔观察每周 3次静滴 1× 10 9/10 0 ml LAK细胞及每日肌注 IL-2 10万 U。结果 观察组总有效率 ( CR+PR)为 7.2 9%( 3 5 /48) ,对照组总有效率 ( CR+PR)为 5 8.3 % ( 2 8/48) ,两组比较相差有显著差性 ( P<0 .0 5 )。观察组 T淋巴细胞治疗前后变化有统计学意义 ( P<0 .0 1) ,而对照组无统计学意义。结论 同种异体 LAK细胞联合化疗及栓塞治疗中晚期肝癌较单纯介入化疗栓塞疗效明显提高 。

Objective To observe the treatment of late stage liver cancer through infusion of LAK/IL 2 into hepatic artery aided with chemotherapeutic drugs. Method 96 cases of middle and late stage liver cancer were randomly divided into two groups: one receiving artery chemotherapy of embolism aided with LAK/IL 2 as the observing group; the other receiving only artery chemotherapy of embolism as the control group. The two groups were both given percutaneous puncture of liver artery, and then infusion of chemotherapeutic drugs was carried out. Result The total effective rate of the observing group was 72.9% (35/48), while that of the control group was 58.3% (28/48)The difference was significant (P<0.05).The change of T lymphocytes in the observing group had statistical value (P< 0.01 ) while that in the control group showed no statistical value. Conclusion Chemotherapy and embolism combined with homologuous LAK cells for the treatment of liver cancer have better therapeutic effect than chemotherapy and embolism without LAK.
