进展期胃癌腹腔镜D2根治术的临床分析

Clinical analysis of laparoscopic D2 radical resection for advanced gastric cancer

目的探讨应用腹腔镜行胃癌根治术No.12a组淋巴结清扫的手术方法及治疗效果。方法随机选取2009年9月～2011年9月收治的胃癌患者60例,其中将行腹腔镜胃癌根治术的30例患者作为观察组,同期行开腹胃癌根治术患者30例作为对照组,对两组患者的手术治疗效果及术后情况进行对比分析。结果观察组患者均于腹腔镜下顺利完成手术,其中行全胃切除术16例,远端胃切除术14例,均行No.12a组淋巴结清扫,其各评价指标(除清扫淋巴结外)均优于对照组(均P<0.05)。结论进展期胃癌行腹腔镜D2根治术,疗效显著,具有微创性、安全性、可行性,符合现代微创外科的需求。

Objective To investigate No.12a lymph node dissection method and therapeutic effect of laparoscopic radical operation of gastric cancer.Methods 60 cases of patients with gastric cancer in our hospital were selected from September 2009 to September 2011,30 cases were given laparoscopic radical operation of gastric cancer,as the observation group;at the same time undergoing open radical operation of gastric cancer patients with 30 cases as control group,operation thera peutic effects and postoperative condition in the patients of two groups were analyzed and compared.Results The patients of the observation group had the laparoscopic operation smoothly,total gastrectomy was 16 cases,distal gastrectomy was 14 cases,they all were treated with group No.12a lymph node dissection,the evaluation indexes（expect lymph node dissec tion） were better than those of the control group（P0.05）.Conclusion Laparoscopic D2 radical resection for advanced gastric cancer has obvious curative effect,minimally invasive,safety,feasibility,and accords with the demand of modern minimally invasive surgery.
