3D-CTA在乳癌术后胸壁放射性溃疡轴型皮瓣移植修复中的临床应用 被引量:2

摘要 目的:探讨运用三维计算机断层扫描血管造影(three—dimensional computed tomography angiography,3D-CTA)现代影像学技术进行术前检查,拟行乳腺癌术后放射性溃疡皮瓣修复手术,明确轴型皮瓣血供,以精确化修复溃疡创面,减少手术并发症,提高成功率。方法:对拟行皮瓣术式修复胸壁溃疡的18例患者行术前3D-CTA检查,根据造影结果,选择手术方式及术前皮瓣设计。结果:3D-CTA能清楚地显示知名轴型动脉,本组18例患者,腹壁上下动脉8例,胸背动脉5例,胸廓内动脉穿支3例,胸肩峰动脉2例。其中16例患者一期手术完成,皮瓣血运良好,全部成活,1例患者行两次皮瓣手术修复,首次TRAM皮瓣远端局部淤紫坏死,二次再行LDM皮瓣修复残余创面,二次手术皮瓣成活良好,治愈出院。另1例患者术前探查双侧胸廓内动脉及下行血管栓塞,调整手术方案,因溃疡面未深及肋骨表面,予以清创植皮修复。结论:借助3D-CTA影像学技术检查,可以更合理地制定轴型皮瓣切取设计方式,简化手术步骤,缩短手术时间,降低手术并发症,提高了手术安全性。
