鼻内镜手术切除鼻腔鼻窦内翻性乳头状瘤的临床研究

Clinical research on nasal endoscopic operation resection of nasal cavity and paranasal sinuses inverted papilloma

目的探讨鼻内镜手术切除鼻腔鼻窦内翻性乳头状瘤的临床疗效。方法选取本院2010年1月～2011年8月收治的鼻腔鼻窦内翻性乳头状瘤患者39例,均行鼻内镜手术切除,术后随访10～18个月,分析患者的临床疗效,并与传统鼻部手术治疗的40例患者相比较。结果 39例患者平均随访(12.6±4.3)个月,37例患者的手术成功切除了病变组织,2例患者术后复发,术后复发率为5.1%,复发时间为8个月和14个月,行二次手术成功切除后,均治愈。鼻内镜手术组手术时间、术中出血量均明显小于传统鼻部手术组,差异均有统计学意义(P<0.05)。鼻内镜手术组并发症发生率(0.0%)低于传统鼻部手术组(5.0%),鼻内镜手术组手术成功率(94.9%)高于传统鼻部手术组(87.5%),差异均无统计学意义(P>0.05)。结论鼻内镜手术切除鼻腔鼻窦内翻性乳头状瘤具有较高的临床疗效,可以明显改善患者的临床指征,具有微创、美容等优点,且术后复发率较低,值得临床推广使用。

Objective To investigate the clinical effect of nasal endoscopic operation resection in nasal cavity and paranasal sinuses inverted papilloma.Methods Thirty nine patients with nasal cavity and paranasal sinuses inverted papilloma were selected in the hospital from January 2010 to August 2011,all patients were given nasal endoscopic operation resection and followed up for 10-18 months after operation.Clinical effect was the analyzed and compared with 40 patients treated with traditional nasal surgery.Results The average follow-up time of 39 patients was（12.6±4.3） months.In 37 patients,diseased tissues were successful resection in operation.2 patients had postoperative relapse,the recurrence rate was 5.1%,The recurrence time were 8 months and 14 months.After two successful operation resection,patients were cured.Operation time,intraoperative blood loss in the nasal endoscopic operation group were significantly less than the traditional nasal operation group,the differences were statistically significant（P 〈0.05）.Complication rate in the nasal endoscopic operation group（0.0%） was lower than the traditional nasal operation group（5.0%）.Operation success rate in the nasal endoscopic operation group（94.9%） was higher than the traditional nasal operation group（87.5%）,the differences were not statistically significant（P 〈0.05）.Conclusion Clinical effect of nasal endoscopic operation resection of nasal cavity and paranasal sinuses inverted papilloma is high,which can significantly improve clinical indications of patients and have advantages of minimally invasive,beauty.The recurrence rate is low,so it is worthy of clinical use.
