网织红细胞参数在非小细胞肺癌化疗前后的变化及临床意义

Change and clinical significance of reticulocyte parameters in non-small cell lung cancer before and after chemotherapy

目的:研究非小细胞肺癌患者化疗前后网织红细胞参数的变化及临床意义。方法:184例非小细胞肺癌患者,根治术后均接受紫杉醇联合顺铂的化疗方案。采用Sysmex XE-5000全自动血细胞分析仪动态观察患者化疗过程中外周血网织红细胞百分数(RET%)、未成熟网织红细胞指数(IRF%)、白细胞计数(WBC)和血小板计数(PLT)的变化。结果:化疗后第5、10天WBC和PLT进行性明显下降(P<0.01),第14天较第10天已逐渐回升(P<0.05),但仍明显低于化疗前水平(P<0.01);而RET%和IRF%第5、10天较化疗前明显下降(P<0.01),RET%第10天较第5天低,IRF%第10天较第5天已有所回升,但两个参数第10天较第5天均无统计学差异(P>0.05),第14天恢复到化疗前水平(P>0.05)。结论:网织红细胞参数可作为监测非小细胞肺癌患者化疗后骨髓抑制和恢复较好的指标,比白细胞和血小板更灵敏,值得临床应用。

Objective： To study the change of the reticulocyte parameters and clinical significance of the non-small cell lung cancer be- fore and after chemotherapy. Methods： 184 patients in non-small cell lung cancer, all radical surgery accepted with paclitaxel and cis- platin chemotherapy. The process of chemotherapy reticulocyte （RET%）, immature reticulocyte fraction （IRF%）, white blood cell count （WBC） and platelet count （PLT） changes were observed by automated hematology analyzer XE-5000. Results： On the 5th and 10th day after chemotherapy, the WBC and PLT decreased significantly gradually （P〈0.01）, on the 14th day it rebounded gradually as compared with that on thel0th day （P〈0.05）,but was still lower significantly than that before chemotherapy （P〈0.01）; On the 5th and lOth day, RET% and IRF% declined significantly than before that chemotherapy （P〈0.01）, RET% on the 10th was lower than that on the 5th day, IRF% rose on the lOth than on 5th day, but two parameters on the lOth day had no significant differences than those on the 5th day （P〉0.05）, on the 14th day return to the level before the chemotherapy （P〉0.05）. Conclusion： Reticulocyte parameters can be used as good index monitoring, suppression and restore bone marrow in the non-small cell lung cancer chemotherapy, it is more sensitive than WBC and PLT, and worth in clinic.
