注射用两性霉素B脂质体致呼吸困难

One case of decompensation caused by amphotericin B liposome for injection

1例7岁女性儿童,确诊急性淋巴细胞白血病3年,既往无药物和食物过敏史。治疗期间在第一次使用注射用两性霉素B脂质体30 min后,出现呼吸困难、寒战、体温39℃,无头痛、低血压。立即停止输注注射用两性霉素B脂质体,给予盐酸异丙嗪注射液10 mg肌肉注射、注射用赖氨匹林0.45 g入莫菲氏管,30 min后症状缓解,4 h后体温降至正常,5 h后继续使用该药物,未再发生类似反应。

One 7-year-old female patient without drug and food allergy history was diagnosed with acute lymphoblastic leukemia three years ago. Thirty minutes after the first use of amphotericin B liposome for injection, she developed dyspnea, shivering and hyperpyrexia （with the temperature of 39 ~2）, while no headache and low blood pressure were found. Amphotericin B liposome for injection was withdrawn immediately. She received the intramuscular injection of promethazine hydrochloride injection 10 mg and intravenous injection of lysine-aspirin 0.45 g. After 30 minutes, her symptoms had relief, 4 hours later, the temperature dropped to normal. While this drug was used again after 5 hours, no similar reaction happened.
