电话指导在坠落伤急救中的价值

Value of phone guidance for first aid in falling injury

目的探讨急救医学电话指导在坠落伤现场急救中的价值。方法 361例高处坠落的患者分为两组,常规组(179例)接到调度指令后赶往现场急救,指导组(182例)在接到调度指令后,急救医生第一时间用车载电话与患方取得联系,按照坠落伤急救电话指导路径指导现场急救,观察分析两组现场急救的正确率、患者现场死亡率、入院后28天死亡率。结果两组患者的现场急救正确率、患者现场死亡率有显著性差异(P<0.01),入院后28天死亡率也有明显差异(P<0.05),指导组明显优于常规组。结论对坠落伤患者进行急救医学电话指导,能大大提高现场急救的正确率,减少现场的盲目处理,降低患者的现场死亡率,提高患者存活率,为进一步的抢救赢得宝贵时间。

Objective To explore the value of phone guidance for first aid in falling injury. Methods 361 cases of patients with falling injury were divided into two groups. In routine group （n=179）, doctors rushed to spot for first aid after getting instructions. In guidance group （n=182）, doctors contact the patients with a phone in ambulance when get dispatch instructions, according phone-guidance-path of falling injury to guide first aid in spot. The first aid accuracy, the mortality in spot and in hospital after 28 days of two groups were observed and analyzed. Results First aid accuracy and the mortality in spot of two groups were significantly different （P〈0.01）. The mortality in hospital after 28 days of the guidance group was significantly lower than that of routing gorup （P〈0.05）. Conclusion Guiding the patients of falling injury by phone for first aid, can greatly improve the accuracy of the first aid, decrease the blind processing in spot, reduce the mortality in spot, improve the survival rate and get more time for fur- ther rescue.
