摘要
目的探讨64层螺旋CT最小密度投影(MinIP)结合CT仿真内窥镜(CTVE)对支气管异物的应用价值。方法对60例临床拟诊为气管支气管异物患者行64层螺旋CT检查,分析64层螺旋CTMinIP结合CTVE等多种重建技术对支气管异物显示情况,并与纤维支气管镜检查结果对照。结果 60例患者中,MinIP结合CTVE技术诊断气管支气管异物39例,以纤维支气管镜为标准,敏感性为94.35%,特异性为95.54%,诊断准确率92.86%;两种方法对支气管异物的检出率比较其差异具有统计学意义(χ2=0.174,P=0.676>0.05)。结论 MinIP结合CTVE技术是一种快速无创的检出方法,大大提高了气管支气管异物的的敏感性、特异性和检出率,对气管支气管异物纤维支气管镜取出治疗有重要价值。
Objective To expore the value of MinlP conlbined CTVE of 64 slice spiral CT in the application of tracheobroqchial foreign bodies in children. Methods 60 patients with tracheobronchial foreign bodies pseudo-diagnosed by clinic underwent 64-SSCT and bronchoscope, and the findings of MinIP combined CTVE of 64 slice spiral CT were compared with that of bronchoscope. Results Of all the 60 patients, 39 cases were found tracheobronchial foreign bodies by MinIP combined CTVE of 64 slice spiral CT. According to "gold standard" of bronchoscope, the sensibility, specificity and accuracy rating of MinlP combined CTVE were 94.35%, 95.54% and 92.86% respectively. The difference in detection rate between MinlP combined CTVE and bronchoscope has no significance (x-'=0. 174, P=0.676〉0.05). Conclusion MinlP combined CTVE of 64 slice spiral CT is a quick and harmless examination. To the patients with tracheobronchial foreign bodies, the sensibility, specificity and accuracy rating were higher and it has important value for the treetment of bronchoscope.
