摘要
目的探讨弥散加权成像序列(DWI)在髋关节软骨中的应用价值;在b=1 000时,测量正常成年人髋关节软骨及邻近骨质的ADC正常值及其范围。方法采用GE HDxt3.0T超导全身磁共振成像仪对103例正常成年人206个髋关节行常规序列及DWI序列成像。测量髋关节软骨各个感兴趣区(ROI)及邻近骨质的表观扩散系数(ADC)值,比较各组间ADC值的差异。结果正常成年人髋关节软骨ADC均值为(10.89±1.66)×10-4 mm2/s,男性和女性的髋关节软骨ADC值无显著统计学差异。同侧髋关节软骨前侧与后侧、外侧与后侧间ADC值有统计学差异(P<0.05)。性别一致时,左右两侧的髋关节软骨ADC值也无明显统计学差异。结论 DWI对关节软骨反应灵敏,正常关节软骨ADC值的测量能够为早期软骨病变的诊断提供参考标准,同时也能够为治疗后的随访评估提供定量的参考指标。
Objective To study the clinical significance of diffusion weighted imaging (DWI) in hip cartilage, and to measure the apparent diffusion coefficient (ADC) values (b 1 000) normal range in normal adults hip cartilage and pe- ripheral bone. Methods 103 healthy adults(206 hips) underwent conventional MR sequence and DWI image with GE HDxt3.0 T MR. The regions of Interest (ROD ADC value of hip cartilage and peripheral bone were measured, the differ ences of ADC values in the groups were compared. Results The ADC mean values of normal adults hip cartilage was ( 10.89 + 1.66) )〈 104 mm2/s, there was no statistical significant difference between male and female. But there was statis- tical difference of ADC values between anterior and posterior in ipsilateral hip cartilage, at the same time, there was statis tical difference of ADC values between lateral and posterior in ipsilateral hip cartilage ( P d0.05). With the same gender, there was no statistical significant difference between left side and right side. Conclusion DWI is sensitive for showing ar- ticular cartilage. The ADC value measurement of normal articular cartilage can be used as the reference standard for the early stage articular cartilage lesions. It can also provide a quantitative reference indicators for follow up assessment after treatment.
