小儿慢性胃炎锝标记固体餐胃排空检测

Measurements of gastric emptying with technetium -labeled solid meal in children with chronic gastritis

目的 通过锝化合物标记的固体餐检测胃排空 ,了解慢性胃炎及幽门螺杆菌 (Hp)感染患儿的胃排空情况。方法 将 30例慢性胃炎患儿按悉尼分类法分为轻度、中度、重度慢性胃炎三组 ;按病理检查结果分为Hp阳性和阴性两组。采用锝化合物标记鸡肝为试餐 ,对各组患儿进行核素检测分析。结果 轻度慢性胃炎患儿的胃半排空时间 (GET1/ 2 )为 5 4.80min ,95 %可信区间 (95 %CI)为37.49～ 80 .2 2min ;中度者GET1/ 2 为 91.2 2min ,95 %CI为 71.88～ 119.49min ;重度者GET1/ 2 大于12 0min。Hp感染阳性的慢性胃炎患儿的GET1/ 2 大于 12 0min ,Hp感染阴性的慢性胃炎患儿的GET1/ 2 为6 1.46min ,95 %CI为 5 2 .70～ 73.92min。结论 慢性胃炎患儿的固体餐GET1/ 2 延缓并随病情加重而延长 ,不同程度病变间有显著差异 ;慢性胃炎患儿是否感染Hp其GET1/ 2 有显著差异 ,Hp感染阳性慢性胃炎患儿的GET1/ 2 明显延迟。

Objective To investigate the status of gastric emptying in children with chronic gastritis and Helicobacter pylori (Hp) infection. Methods Thirty children with chronic gastritis confirmed by biopsy pathology were classified into mild,moderate and severe groups and in Hp positive and negative groups, each child was tested by single photon emission computed tomography (SPECT) imaging with a technetium compound ( 99m TcO - 4) labeled chicken liver solid food and the characteristics of gastric emptying were analyzed. Results The half gastric emptying time (GET 1/2 ) in mild,moderate and severe chronic gastritis groups was 54.80 min, 91.22 min and >120 min, respectively, the 95% confidence interval (95%CI) of mild and moderate chronic gastritis groups was 37.49 80.22 min and 71.88 119.49 min, respectively. GET 1/2 surpassed 120 min in Hp positive group, but was 61.46 min in Hp negative group, 95%CI was 52.70 73.92 min. Conclusion The results indicated that GET 1/2 of children with chronic gastritis was prolonged with pathological severity; significant difference was found among mild, moderate and severe groups. GET 1/2 in chronic gastritis children with Hp infection was prolonged.
