青光眼小梁切除术中两种不同结膜瓣 缝合方法疗效观察

Special conjunctival sutures in the glaucomatous filtering surgery

了解青光眼小梁切除术中，对以穹窿部为基底的结膜瓣采用两种不同缝合方法的手术效果，观察滤泡形态、治疗效果及并发症。方法：两组维吾尔族闭角型青光眼病人每组２５只眼，共５０只眼。均施行小梁切除术，采用以窟窿部为基底的结膜瓣。其中一组仅将结膜瓣对位间断２～３针缝合在角膜缘处。另一组将结膜瓣行连续锁边缝合，成为一密不漏水的结膜瓣。结果：结膜瓣连续锁边缝合组形成功能性滤过泡的比率明显大于未对结膜瓣做特殊处理组。结论：小梁切除术中，结膜瓣关闭时达到绝对不漏水是手术成功的关键，特别是当前广泛使用抗代谢药物的情况下更是如此。

Objective: To observe the clinical effects and complications and the filtering bleb during the trabeculectomy surgery by means of sewing conjunctiva with different ways in the superior fornix-based peritomy. Methods: The study was randomized and double-masked. 50 Uighur persons with closed-angle glaucoma were placed into two groups. Each group contains 25 cases. Trabeculectomy were performed in both groups .Among them, one group sewed conjunctiva with successive chain shaped sutures, and another used common suture technics. Results: The rate of successful filtering bleb in special suture groups was higher than the group using common technics. conclusion: It is suggested that the spescific conjunctival suture was more effective than the nonspecific conjunctival suture in the glaucomatous filtering surgery.
