甲状腺功能亢进性心脏病病人的围手术期处理

Perioperative management of hyperthyroid heart disease.

目的 总结甲状腺功能亢进 (甲亢 )性心脏病的围手术期处理经验。方法 回顾性分析 76例甲亢性心脏病的围手术期药物治疗方法。结果 全部病人均顺利度过围手术期 ,无围手术期死亡 ,无甲亢危象及心脏功能障碍发生 ,经抗甲状腺药物及甲状腺次全切除术的治疗 ,76 % (5 8/ 76 )的病人围手术期内甲亢性心脏病获得治愈。结论 甲亢性心脏病并非手术的禁忌证 ,经抗甲状腺药物治疗及恰当术前准备 。

Objective To summarize the experience of perioperative management for hyperthyroid heart diesase.Methods Retrospectively analyze the treating experience of 76 cases of hyperthyriod heart diesase.Results All patients safely went through subtotal thyroidectomy,no perioperative death,no thyroid crisis and heart failure,76%(58/76) hyperthyroid heart disease were cured after operation.Conclusion Hyperthyroid heart disease is not the contraindication of subtotal thyroidectomy.With the treatment of antithyroid drugs and proper preoperative preparations,most of the heart disease can be controlled effectively and the patients can go safely through operation.
