转变农村基层医疗卫生机构运行机制是缓解农民"看病贵、看病难"的关键,是"保基本、强基层、建机制"的重要抓手和切入点。调查显示,"以药养医"运行旧机制已基本破除,但是"维护公益性、调动积极性、保障可持续"的运行新机制尚未建立起来,农民"看病贵、看病难"问题依然不同程度地存在。农村基层医疗卫生机构改革是一项综合性工程,只有进行综合、系统、全面地改革,才能建立起新的体制机制。因此,推进与基本药物制度相配套的一系列体制机制综合改革,已成为当前十分紧迫的重要任务。

The key to relieve ＂difficulty and high cost in seeking medical services for peasants is to transform rural community medical institutions,and it is also an important handle and breakthrough point for ＂guaranteeing basic service,strengthening community capability and establishing mechanism＂.Survey shows that compensation mechanism for medical institutions by medicine-selling has been roughly eliminated.However, the new operating mechanism of maintaining public-welfare,stimulating enthusiasm and guaranteeing sustainability is not established yet.The issue of ＂difficulty and high cost in seeking medical services＂ still exists in different degrees.It is a comprehensive project to reform rural community medical institutions,and only carrying out comprehensive,systematic and complete reform,can new system and mechanism be established.Therefore,it has become an urgent mission to promote a series of comprehensive system and mechanism reforms which are coordinated with basic pharmaceutical reform.
