地佐辛复合丙泊酚用于无痛人流的临床研究

Clinical research on effect of dezocine combined with propofol on painless abortion

目的探讨地佐辛复合丙泊酚用于无痛人工流产(人流)的临床麻醉效果和安全性。方法将美国麻醉医师协会(ASA)分级Ⅰ级的200例自愿终止妊娠的孕妇随机分为两组,每组100例。实验组:30 s内匀速静脉注射地佐辛5 mg;对照组:30 s内匀速静脉注射芬太尼1μg/kg。两组均在静脉滴注药物10 min后以丙泊酚2 mg/kg进行静脉麻醉。观察比较两组患者生命体征、麻醉诱导时间、术中体动次数、苏醒时间、术后镇痛效果及不良反应方面的差异。结果对照组较实验组呼吸抑制明显(P<0.05);两组麻醉诱导时间、术中体动次数、苏醒时间比较,差异无统计学意义(P>0.05);而术后镇痛效果及不良反应实验组均优于对照组(P<0.05)。结论地佐辛复合丙泊酚用于无痛人流镇痛效果确切,呼吸抑制轻,术后苏醒迅速,不良反应少,值得临床推广应用。

Objective To investigate the clinical anesthesia effect and safety of dezocine combined with propofol on painless abortion.Methods According to the American Society of Anesthesiologists（ASA） classification,200 pregnant women of grade Ⅰvoluntary termination of pregnancy were randomly divided into 2 groups with 100 cases in each group.The experimental group was intravenously injected with dezocine 5 mg within 30 s,while the control group was intravenously injected with fentanyl 1 μg/kg within 30 s.At 10 min after injection,the two groups were performed intravenous anesthesia by infusion of propofol 2 mg/kg.The differences in vital signs,time of anaesthesia induction,intraoperative body movement times,recovery time,postoperative analgesia effects and adverse reactions were observed and compared between the two groups.Results The degree of respiratory inhibition in the control group was much more obvious than that in the experimental group（P〈0.05）.There were no statistical diference in time of anaesthesia induction,intraoperative body movement times,recovery time between both groups（P〈0.05）.The postoperative analgesia effects and the adverse reactions in the experimental group were much better than those in the control group（P〈0.05）.Conclusion Dezocine combined with propofol has good effect with low degree of respiratory depression,rapid postoperative recovery and fewer adverse reactions in painless abortion,and is worthy of clinical application.
