听神经瘤手术治疗的临床分析

Clinical analysis on surgical treatment of acoustic neuroma

目的分析与探讨听神经瘤的手术治疗方法与效果。方法选取2008年1月至2011年1月收治的听神经瘤患者42例,对其临床资料进行回顾性分析,所有患者均采用枕下乙状窦后经过内听道入路接受显微手术。结果42例患者中,采取听神经瘤全切共38例(90.5%),采取听神经瘤次全切共4例(9.5%)。面神经解剖保留共31例(73.8%),术后1年面神经功能保留共35例(83.3%)。结论对于听神经瘤进行手术治疗的过程中,医生能够熟练显微手术的技巧、掌握桥脑小脑角解剖,对于完成肿瘤的切除以及减少面神经的损伤有重要的意义。

Objective To analyze and discuss the methods and effects of acoustic neuroma operation treatment.Methods 42 cases of acoustic neuroma were treated by operation from January 2008 to January 2011 and their clinical data were retrospectively analyzed.all patients received microsurgical operation by suboccipital retrosigmoid approach through the internal auditory meatus.Results Among 42 cases of acoustic neuroma,38 cases（90.5%） adopted acoustic neuroma total resection,accounting for 90.5%,4 cases adopted acoustic neuroma subtotal resection,accounting for 9.5%.Facial nerve anatomy retaining was in 31 cases（73.8%）.The functional preservation of the facial nerve in postoperative 1 year was in 35 cases（83.3%）.Conclusion During the operative course of acoustic neuromas,the doctor can master the microsurgical operation skills and the cerebellopontine angle anatomy,which is important for completing tumor excision and reducing facial nerve injury.
