椎体成形术和椎弓根螺丝钉固定治疗椎体压缩性骨折疗效评价

Comparing the Clinical Experience of Spinal Vertebroplasty and Pedicle Internal Fixation on Patients with Fracture of Vertebral Compression

目的:探讨使用椎体成形术和椎弓根螺丝钉治疗胸腰椎椎体压缩性骨折的临床疗效。方法:2006年9月至2012年3月间我科共收治椎体压缩性骨折136例。采用随机分组的方法,将136例患者分为两组,两组患者各项身体指标之间无差异(P>0.05)。两组各68例,其中治疗组胸椎骨折20例,腰椎骨折48例,手术方式采用椎体成形术;对照组患者中胸椎骨折18例,腰椎骨折50例,手术方式采用脊柱后路切开复位椎弓根内固定术,观察并分析比较两组之间的差异。对两组患者手术前后的VAS(视觉疼痛评估),椎体前缘高度和Cobb角分别进行评估分析。结果:本研究发现两组患者术前情况(疼痛、椎体高度和Cobb角)比较无明显差异(P>0.05)。手术后患者疼痛明显减轻,椎体前缘高度恢复良好、Cobb角明显改善(P<0.05),并且发现治疗组效果与对照组相比恢复得更好(P<0.05)。结论:经皮椎体成形术有效改善了患者的生活质量,取得了满意疗效,值得推广应用。

Objective： To explore the effect of vertebroplasty to compression fracture of thoracic and lumbar vertebral body. Method： Selecting 136 cases with vertebral compression fracture in our department from Sep. 2006 to Mar. 2012. Divided 136 eases into two groups randomly, There＇s no significant difference in health indicators of two groups （ P〉0.05 ）. Treatment group had 20 cases were fracture of thoracic verte- bra, 48 cases fracture of lumbar vertebra, treated by vertebroplasty. Control group had 18 cases fracture of thoracic vertebra, 50 cases fracture of lumbar vertebra, treated by open reduction with pedicle internal fixa- tion of posterior spinal. Compared the treatment effectiveness . Analysis VAS of before operation, height of vertebral body front , Cobb angle of two groups. Result： Before operation, data of two groups don＇ t have a significant difference （ P〉0.05 ）. After operation , pain was significantly reduced, height of vertebral body front and Cobb angle were significantly improved （ P〈0.05 ）. Treatment effect of treatment group was better than control group （ P〈0.05 ）. Conclusion： For patients, pereutaneous vertebroplasty can effectively improve quality of life, have a good treatment effect. It is worthy of popularization and application.
